Metered usage now requires a payment method

Paid plans can run past included credits and credit packs with on-demand metered usage — but only when billing is ready to charge your organization.
What changed
- Payment method required — metered overage is effectively off until Stripe reports a valid payment method on your org customer. The Usage page shows a warning and a setup button when payment is missing.
- Org + project toggles — both organization and project on-demand metered switches must be on. Each project can opt out without affecting others.
- Org-first budgets — when monthly spend caps apply, organization hard-stops run before project caps, so org limits always win.
Included subscription credits and one-time credit packs are unchanged.
