Metered usage is now opt-in

VoiceThere now treats on-demand metered usage as an explicit opt-in. New organizations and projects no longer start with metered overage enabled by default.
What changed
- Included credits and packs work the same as before.
- Metered overage (billing past included + packs through Stripe) stays off until you add a payment method and turn on the org and project toggles.
- Dashboard switches are disabled until billing is configured, with a clear hint on the Usage page.

On Organization → Usage, add a payment method, then enable On-demand metered usage when you are ready for pay-as-you-go billing after included credits and packs.
